“The Democracy Amendment”
“The Senate of the United States shall be permanently dissolved. All functions of the Senate shall be absorbed by the House of Representatives, except that the Vice President of the United States shall have no position in the House.
The President of the United States of America and the Vice President shall be elected by a majority vote from citizens in the country as a whole. The House of Representatives may determine the time of voting for the President and Vice President and direct national voting standards for their election, and for election to the House, to be supervised by Officers of the United States as provided for by Law.”
That should do the trick. Keep in mind that under Article V of the Constitution two thirds of the states can convene a constitutional convention and that proposed amendments are valid upon approval by three fourths of the states. It's true, this route for amending the Constitution has never been tried, but it may well be the only remaining way to break the lock of corporate money on our government.

"The right of Free Speech, guaranteed in the First Amendment to the Constitution of the United States is reserved for the citizens and is neither guaranteed to nor a right of corporate entities."
This may be as important or more so than your other two changes though I do like them a lot. Unless we destroy the fiction of corporate entities as persons we are unable to convince our judicial system to enforce campaign finance laws etc.
